The person completing this form must be the designated responsible person for all team bookings, cancellations and payments.
 
In Section 1 you will need to complete the booking details for your team and your contact details.
 
In Section 2 you will be asked to attach your insurance documents.
 
In Section 3 you will be asked to provide details of your safeguarding officer/lead and attach your safeguarding policy/procedure.
 
In Section 4 you will be required to read and acknowledge Havant Borough Council's terms and conditions before continuing this form.
